Middlebury Office:

New Milford Office:
Friday: Noon to 4:00 P.M.

Farmington Office:
Wednesday and Thursdays:
9:00 A.M. to 5:00 P.M.

Appointments

Appointments are required. Call us at 203-598-0700 to schedule one. We are sorry that we cannot accommodate walk in appointments. Please give 24 hour notice if you are not able to keep a scheduled appointment.

Please advise us if there has been any change in your address, phone number or insurance coverage since your last appointment.

Please remember to bring to your appointment:

  • Your completed paperwork
  • Insurance cards
  • A photo ID or driver’s license
  • A co-pay if your insurance requires one
  • Any applicable X-rays, MRI’s, or other medical records
  • A pharmacy card if you have one
  • A list of your current medications
  • Wear loose, comfortable clothing or bring a pair of shorts

Payments

Payment is expected when services are rendered unless prior financial arrangements have been made.

HMO Subscribers

Any HMO required referrals are the patient's responsibility. If your insurance requires a referral and you do not have a referral for your office visit, you will be asked to reschedule your appointment.

Insurance cards and copays are required at time of service.
As a courtesy to our patients, we will bill any commercial insurance carrier assigned, but the patient remains financially responsible for all services furnished. Any outstanding balance after 60 days from the date of invoice is considered in default.

Workers' Comp patients must bring all applicable insurance information to initial appointment including:

  • Billing address and phone number
  • Adjuster name and phone number
  • Claim number
